Set-up and equipping of Children's Residence House

Set-up and equipping the residence house for 10 ill children along with his/her mother with the intention of providing accommodation as well as providing special treatments for the patients under the cover of Zanjireh Omid Charity institute who are living in provinces which there are no standard medical treatments available. In this center, treatments and special care for before and after surgeries will be provided. Furthermore, launching a physiotherapy clinic according to the pediatric protocols as well as creating a clinic for the lip and cleft palate for the Zanjireh Omid charity institute.

In this center, treatments and special care is given to patients before and after the surgery.


A. Project's Description


The projects consist of three parts: A.1. Residential Section A.2. Medical Section A.3. Administrative Section. The total area of the center is about 800 square meters.

A.1 Residential Section


Accommodation of 10 children with their mother


This section includes 2 parts. The first section comprises of the hospitalization beds for the children and their mothers which will be separated by the partitions. This section will be located on the first floor. The second part involves, dining room, kitchen, storage, prayer room, TV room, laundry and a training room which are located on the basement floor.

A2. Medical Section

The main purpose of this section is to allocate a space for additional treatments before and after the surgeries. This section of the project consists of three parts:

A2.1 Diagnosis and treatment section – This section has an examination room for children in the three fields of cardiac, orthopedic and reconstructive in order to hold specialized clinics and two separate rooms to hold the Cleft palate clinics.

A2.2. Physiotherapy and Rehabilitation clinic - In this section, rehabilitation and physiotherapy for before and after surgeries are provided to the pediatric patients. This section consists of physiotherapy, occupational and speech therapy which includes electrotherapy and mechano-therapy.

A2.3 Specialized educational training section such as how to provide care for children with cardiac orthopedic and reconstructive problems and children being hyperactive, general educational trainings such as life skills and skills needed in order to prevent from social problems. Rehabilitation such as primary educations in order to prevent from academic failure of school children.

A2.4 Training for primary medical health for women

A2.5 Creation of the cleft lip and palate unit, creation of visiting unit as well as dentistry unit located on the third floor of center in order to proceed with the lip and cleft palate project along with the doctors of the Zanjireh Omid charity institute.

A.3 Administration and personnel section

A3.1 In the residential Section






Personnel: 4 Nurses (working on shifts), 1 Maintenance person

Service section in the residential unit: Personnel: 1 chef, 2 chef assistants, 1 maintenance person, 1 person for storage and 1 person in charge of the laundry room AC3.2 In the Medical Section


Physiotherapy section - Personnel: 2 physiotherapists, 1 occupational therapy experts, 1 speech therapy expert, 1 Maintenance person, 1 financial individual, 1 receptionist

Cleft lip and palate - Personnel: 1 experienced individual in order to carry out the lip and cleft palate project 
A.3.3 In the Administrative Section


Personnel: 1 person responsible for administrative support, 1 receptionist, 3 security guards (working on shifts)


B.  Project's Goals

Medical Goals


Considering that Zanjireh Omid institute provides special care and treatment to the underprivileged children, and as such treatments requires long term accommodation before and after surgeries, constructing of the residential Centre will have the following benefits:

1.   Supervision of Zanjireh Omid doctors before and after the children's surgery will lead to better results
2.   Reduction of hospitalization infections by decreasing the length of hospitalization of the children
3.   Implementing care plans for before and after the surgery and completion of the treatment process
4.   Increasing the space required for children who need long term care

5.   Reducing the physiotherapy costs which involves great amount of the medical costs as well as providing the best occupational therapy services according to the pediatric protocols for the children under the cover of Zanjireh Omid Charity institute

Social Goals

As the patients who are under the cover of Zanjireh Omid are mostly from lower classes of the society and they travel to Tehran for the treatment services, construction of this Centre will have various social benefits. The benefits are as follow

1.   Reduction of the hospital costs due to the decreased hospitalization of the children in the hospital
2.   Due to the financial problems which the families from other provinces have and their problem with staying in Tehran, families usually do not continue their children's treatment process and return to their home city. As a result, this Centre will provide the children and the mother accommodation during and after their treatment process.
3.   Provision of a safe psychological environment for the patients

4.   Provision of education to boys and girls by teaching primary courses in order to avoid their failure in their education
5.   Educating parents through short term classes such as basic life skills, social harms and the ways to provide care for the children after their surgery

The most important characteristic of this project is to achieve the best form of treatment with considering the treatment health care costs.




In this regard, the following steps will be performed for each patient
Establishment of specialized clinics by the member doctors of Zanjireh Omid
Referral of the orthopedic patients before and after surgery for physiotherapy, for the patients who need surgery
Referral of the patients according to the doctor's instructions in patients who do not require surgery
Examination and re-visit during the treatment process, to follow their treatment process by the doctors of Zanjireh Omid

C. Project's Beneficiaries


All of the deprived children under the age of 18 who need to have orthopedic, cardiac and reconstructive surgery and are living in provinces, can benefit from the children's residence house services. Also, the children who are living in Tehran can also benefit from paramedical and medical services of this project.


It should be noted that, physiotherapy equipment has been donated from Japan Embassy and Saman Bank and has been purchased which currently it is being used is in the physiotherapy center of Zanjireh Omid in West of Tehran. However, due to the lack of adequate access for the patients, the equipment will be transferred to the new location (children's residence house).

At the end, it should be noted that, with launching this project, Zanjireh Omid institute as well as providing treatment to underprivileged children, it can also develop its ability to expand its services and activities and to be able to be active as well in other specialized areas

Project Beneficiaries:

Children covered by Zanjireh Omid charity institute who are accepted from provinces and their mothers as well.

 Implementation of the project
order to reception and the occupational therapy, speech therapy, physiotherapy and physical therapy sections.

It should be noticed that the rehabilitation services are one of the most important part of orthopedic treatment which the children covered by Zanjireh Omid (the residents of children house or the nonresident ones) should use them.

2- Repair and reconstruction the underground: this section consists of dining hall, kitchen, storage and refrigeration, laundry, children's game room and mother's training room.

3- Repair and reconstruction the first floor: this floor with 14 inpatient bed, 14 seats with folding bed, the nursing room, bathroom, toilets and a nurse countertops acceptance for residents include 14 children and their mothers, was provided to prepare them for surgery.

4- Repair and reconstruction the second floor: this floor is consisting of conference room, server room, office rooms and dental unit. It should be explained that for the cleft lip and palate project and the need of dental and orthodontic clinics, this section is prepared and ready to use.







Medical Goals

Since the Zanjireh Omid Institute provides special care and medical services for unprivileged children, the children have to stay for long in order to pre and post-surgery cares.

Launching the medical dormitory has following results:

1- Doctors supervision of children before and after surgery which lead to better medical results

2- Reduction of nosocomial infections by reducing the time of children stay in hospital.

3- Planning of care programs before and after surgery, and complete the process of treatment

4- Increased space for children who need long-term care 
5-charges.

6- Psychological counseling services


Social Goals

With regard to the fact that patients under cover of this institution are from deprived and down level of the society and come from provinces, the construction of the center has many social benefits along with. These benefits include:

1- Reduce hospital costs due to a reduction in hospitalization of children

2- Due to the families of patient's financial difficulties and lack of accommodation in Tehran, families are generally discouraged from continuing their children treatment and they go back to their cities. The center provides the accommodation for these children and one of their parents.

3- Providing a safe emotional environment for patients

4- Teaching primary lessons to boys and girls in order to avoid interrupting their education, training their parents by holding short-term courses on the subject of life skills, social harms and children's care after surgery.



























Providing Running Costs of Children’s House in Order to Offer Therapeutic Services Before and After Surgery to Underprivileged Children


Necessity of the Project
Underprivileged children who receive top healthcare services at the institute come from impoverished areas. They need a place to stay during their treatment, surgical operations and rehabilitation therapy (orthopedic cases); moreover, they require nutritive and healthy diet during their treatment process to restore their health.

Description
Running costs of Children’s house are divided into two parts:
1. Costs of residence and services
2. Costs of medical care and rehabilitation


Residence
There are 30 beds for children and mothers in 4 separate rooms in Residence section. There are 4 beds for cardiac patients in Narges room, and there are 14, 8 and 4 beds in Shaghayegh, Maryam and Sonbol rooms for orthopedic and reconstructive patients. There is a treatment room containing medical equipment and medications in this section. Nursing station and tiolets are also located in Residence unit. Nurses care for patients in various shifts and 24/7.
Meals including snacks, lunch, dinner and breakfast are served 5 times a day. There is a kitchen in Residence section. There is a restaurant, fridge and store room for preserving foods downstairs. playroom, and security room, maintenance and service, laundry, praying room and detergent room are all located in residence section.
Running costs include: restaurant, taxi, courier, stationery, cleaning products, Internet, maintenance, repair, social work unit, transportation, insurance, uniform, playroom, health care, medications, nursing salaries, security guard, kitchen, services, maintenance, 23%
insurance.

Medical care and Rehabilitation
This section includes a clinic room, examination room, dentist’s room, and social work unit. Patients are examined at no cost by specialists after admission by social work unit. The treatment process begins after admission which might include: surgery (pre and post-surgery care) and rehabilitation services including: physiotherapy, occupational and speech therapy. Pre and post-surgical treatments are also offered at rehabilitation unit by medical staff. Other services include: physiotherapy, mechanotherapy, occupational therapy (physical and mental), and speech therapy.
Costs of this section include: admission, clinics, salaries of rehabilitation unit.
